Use this form when a family needs to begin senior living research quickly because of an upcoming discharge, caregiver breakdown, unsafe living situation, or sudden change in support needs.
Priority assistance is for time-sensitive senior living navigation. It is not an emergency, medical, transportation, or crisis-response service.
Calling or submitting this form does not guarantee immediate contact, provider availability, admission, assessment, or a specific move-in date.
Do not submit this form and wait for a response when someone is experiencing immediate danger, serious medical symptoms, a fall with injury, wandering or missing-person concerns, suspected abuse, or another emergency.
For urgent medical questions that are not emergencies, contact the senior’s physician, hospital team, rehabilitation team, home health provider, hospice provider, or appropriate clinical service.
The hospital has provided or is preparing an expected discharge date, and the family needs to research senior living options.
