TIME-SENSITIVE SENIOR LIVING GUIDANCE

Request Priority Assistance

Use this form when a family needs to begin senior living research quickly because of an upcoming discharge, caregiver breakdown, unsafe living situation, or sudden change in support needs.

Priority assistance is for time-sensitive senior living navigation. It is not an emergency, medical, transportation, or crisis-response service.

Calling or submitting this form does not guarantee immediate contact, provider availability, admission, assessment, or a specific move-in date.

Is This an Immediate Medical or Safety Emergency?

Do not submit this form and wait for a response when someone is experiencing immediate danger, serious medical symptoms, a fall with injury, wandering or missing-person concerns, suspected abuse, or another emergency.

Call 911 for immediate emergencies.

For urgent medical questions that are not emergencies, contact the senior’s physician, hospital team, rehabilitation team, home health provider, hospice provider, or appropriate clinical service.

When This Pathway May Be Appropriate

Hospital Discharge

The hospital has provided or is preparing an expected discharge date, and the family needs to research senior living options.

Rehabilitation Discharge

A rehabilitation stay is ending and returning home may not be safe or sustainable.

Caregiver Breakdown

A spouse, adult child, or other caregiver cannot continue providing the current level of care.

Unsafe Home Situation

The current living arrangement may no longer provide adequate supervision, personal care, mobility support, or safety.

Sudden Change in Needs

Memory, mobility, behavior, personal-care, or supervision needs have recently increased.

Current Setting Is No Longer Appropriate

A senior living or care provider has indicated that another setting may need to be considered.

Seniority DFW does not determine whether a senior is medically ready for discharge or which clinical level of care is required.

Tell Us What Is Happening

Provide general information so a Seniority DFW advisor can understand the situation and determine the appropriate next step.

These selections do not determine the appropriate care level. Each receiving provider completes its own assessment.
Seniority DFW may need to clarify authorization before contacting another individual.
Privacy Notice: Do not submit Social Security numbers, Medicare or insurance identification numbers, financial account details, credit card information, passwords, full medical records, detailed medication lists, legal documents, or other highly sensitive information through this form.
